Is the "Add account" form opening on another (not visible) monitor? Try setting
Window | Original sizes in the application and see if that fixes the problem.
Does it freeze for you on your computer? If so, please send me a ZIP of the data, either via email or
http://www.softrak.com/dbr marked to my attention.
Also, you might try installing yesterday's Service Pack and see if that changes things, although there is nothing mentioned in the Release Note about this kind of issue.
