Assuming data integrity is clean, about the only other thing to suggest is to make sure that all users have their own login initials and password, rather than people sharing the same login. SmartFinder access is saved by user and there will be problems if two people try to use SmartFinder on the same field at the same time with the same login code.
Because you indicated it doesn't happen consistantly but only in some occasions, there isn't much we can go on. You may have to try things like what happens if two people try simultaneous lookup of SmartFinder on items, or identifying what steps were taken before the error occured - to determine the steps to make the error repeatable. You may also have to check things like network rights, opportunistic locking settings, and other environment factors.
