Am unable to add new or edit new accounts. Not sure if this is since the recent upgrade to 9.1A as I don't use every day but have been able to do so in the past.

"Unable to add" is a bit vague. Is there an error message? Is there no menu choice?

When I click on "New" to open a new account nothing happens. Same when I click on "edit". It almost appears like it goes in to the background or something. I have to escape to get back in to do anything like to reset.

Is the Window opening on another screen? Do you ever use dual monitors?

Under File | User preferences | General disable "Save window position". Exit Ledger and start it again. See if the Window reappears.

Or, Click "New", then press Alt-Spacebar and "M" to move the window. Move your mouse around to see if the Window appears.

I had a similar issue trying to set-up a new dataset. I had added departments using GVRW with a R/E department of "000". When I went to add the account, when I entered 000, the field went blank when I tabbed out to save the account dept combo. Thus, I it appears that I cannot have a dept with OOO as the dept number? I am not a fan of using 000 as a dept number, but quite a few clients do use this for their balance sheet.

Anyway I forced the change of the account to have a dept "000" with GVRW, and then when I went back to the maintain accounts window, I could not copy new accounts. As indicated by Janet, nothing happened when I clicked on the edit or copy buttons. When I changed the dept back to an acceptable dept, the maintain accounts window buttons worked fine.

Adagio removes leading zeroes from codes (Department, Accounts, Vendors, Customers and so on). It sounds like that is what you are encountering.

No multiple screens and the things you've suggested here are not working. Nothing will work until I escape. The background goes light as though a screen has been opened but nothing. Unlike the note above my copy button does work. Only the new, edit and view don't.
