What formula do I use in a FR spec so that the column heading says (Provisional) if the Provisional "P" icon is toggled on?

In other words, I want the heading to toggle between "Actual" and "Actual (Provisional)" as the icon is toggled.

You only need the IF() because you want the "Provisional" in "()".

This is brilliant! It is always difficult to see whether "Provisional" is on or off. This makes so much sense!
