Thanks for providing the exact error message. I wasn't able to find a record of it being reported before.
What version(release date - in yymmdd format to the right of the version code) of Ledger are you using?
You say you receive the message periodically. Is it always exactly the same message? Are you able to recreate the error, make it happen at will? If so, are you able to recreate the same error using sample data? Or, can you provide exact steps that we can use to recreate the error?
Access violations can often be very difficult to duplicate as there is something that 'occasionally' is occurring on the system that makes a process bump into something else (eg. can't access a file yet as it is being used by some other process). However, if it can be duplicated, we would really like to know so that we can duplicate it also and have R&D look into it.
It never hurts when an error message is encountered to run a Data Integrity Check on the data and make sure there are no errors reported. If there are, make sure you do a backup first, then DIC and Rebuild the errors. Run DIC again to make sure there are no more errors reported.
