Our statements are printing without our name & address on them.

Our company profile is set up with that info.

We can't find any other place to format this information. We must be doing something wrong for it to not include our name.

You need to add fields M3, M4, M5, M6 and M7 to your statement specification.

If you have no idea what I'm talking about, and do not want to spend the time it takes to learn how to design specifications in Adagio, then you should either contact your Adagio consultant, or start with the a default statement specification (the defaults are readily available by launching the sample data).
