You did not say what application you are trying to report on, however:
1. If AR, these fields are in the Customer Information table.
2. If QIP, ph/fax fields can be found in the History Headers table, however, if you have just installed the upgrade for QRW, you will have to reinstall QIP and/or QIWIN for QRW to see the tables for these products.

Order of install for QR/QRW is:
1. ACCPAC Plus modules
2. QR and/or QRW
3. All other 3rd Party applications
4. Quik History (if used).
