We have seen the same issue lots of times for may multicurrency clients. We thought it might be related to the historic transactions or backdated entries into a previously revalued period, but haven't had the time to chase down the exact details.

To fix, you can just do a JE for the current period being revalued (with the Month End Date), with 0$ USD source and adjust the net amount that you want the adjustment to be in $CAD. You have to be careful because the rate will continue to attempt to be the rate in effect for the date, but it is completely possible to do an exchange value entry with zero source $. And also careful if you reopen to edit the entry after saving the batch details because the previous entered amounts will change to match the exchange rates for the transaction date (also an known problem so you will have to reenter your previous entered amounts before you post).
