I saw this happen once. I think it was caused by a flaky network card or a hard drive error. Part of the AR records were lost. The only solution was to go back to the last good backup and re-create the activity after that.
Since the records were missing altogether, DIC did not find an error.
Your consultant might be able to help you find the last good backup by looking at file sizes of the Customer index files.
